Terms & Conditions

In order to qualify for The Dysport Journey, patients must be new to ProMD Health and new to Dysport. This campaign is limited to 3 visits. Each visit is offered at a flat rate of $467 and includes up to 64 units, which is the recommended FDA full dosing. The final number of units administered at each appointment is determined at the provider’s discretion based on individual assessment. The discounted rate applies only within the program. Treatment under the program includes the glabella, frontalis, and crow’s feet. Additional areas outside of these standard treatment areas may be added at the standard rate of $12 per unit.

Each appointment must be completed within 90-120 days of the previous visit, and all three appointments must be completed within 365 days. Failure to complete the series within 365 days will result in forfeiture of the program discount. The Dysport Journey must be completed at the ProMD Health office where treatment was initiated. The program is non-transferable and must be used by the same patient. This program is valid for Dysport only and cannot be applied toward Botox or Daxxify. Other terms may apply.
