Cancellation and Missed Appointment Policy
Our goal is to provide quality individualized medical care in a timely manner. “NO-SHOWS” and late cancellations inconvenience those individuals who need access to medical care in a timely manner. We would like to remind you of our office policy regarding missed appointments. This policy enables us to better utilize available appointments for our patients in need of medical care.
We understand that delays can happen; however, we must try to keep the other patients and the doctor on time.
If a patient is 15 minutes past their scheduled time we will have to reschedule the appointment.
Cancellation of an Appointment
In order to be respectful of the medical needs of other patients, please be courteous and call ProMD Health promptly (24 hours) in advance if you are unable to show up for an appointment. This time will be reallocated to someone who is in need of treatment. If it is necessary to cancel or reschedule your scheduled appointment, we require that you call at least 24 hours in advance. Appointments are in high demand, and your early cancellation will give another patient the possibility to have access to timely medical care.
For this reason, we will need to keep a credit card on file in the event that you do not adhere to our cancellation policy.
How to Cancel Your Appointment
To cancel or reschedule appointments, please call 410-449-2060. If you do not reach the office coordinator you may leave a detailed message on the voice mail. If you would like to reschedule your appointment, please leave your phone number. We will return your call and give you the next available appointment time.
Late Cancellations: A late cancellation is considered to be when a patient fails to cancel their scheduled appointment with at least a 24-hour advance notice.
No Show Policy
A “NO-SHOW” is someone who misses an appointment without canceling it in an adequate manner at least 24 hours in advance. A failure to be present at the time of a scheduled appointment will be recorded in the your medical record as a “NO-SHOW”.
- First missed appointment: a fifty dollar ($50) fee will be billed to your account
- Second missed appointment: a seventy five dollar ($75) fee will be billed to your account
Cancellation / No Show Policy for Procedures
Due to the large block of time needed for Kybella Procedures, PRP “Platelet Rich Plasma” and Fat Transfer Procedures, last minute cancellations can cause problems and added expenses for the office.
Scheduling a Procedure will require a 50% deposit. Payment for the balance of the procedure fee is due at the pre-procedure exam or 7 days prior to the procedure, whichever comes first. If fees are not provided 5 business days before the scheduled procedure date, the procedure may be cancelled.
Procedure deposits and payments are non-refundable.
Fix My Filler Program
ProMD Health offers free reversal for filler that you are unhappy with from another practice or provider. All that is required is a down payment for your replacement filler at ProMD Health.

I understand that I have the right to revoke this authorization at any time and that I have the right to inspect or copy protected health information to be disclosed as described in this document. I understand that a revocation is not effective in cases where the information has already been disclosed, but will be effective going forward.
I understand that information used or disclosed as a result of this authorization may be subject to re-disclosure by the recipient and may no longer be protected by federal or state law.
I understand that I have the right to refuse to sign this authorization and that my treatment will not be conditioned on signing. THIS AUTHORIZATION SHALL BE IN EFFECT UNTIL REVOKED BY THE PATIENT.
